Make erx Evidence of Prescription same size as standard repeat backing

The current size of Evidence of Prescription/Repeat Token is too large to fit onto our repeat backings and it's hard to read what medication each token is for. It also makes the prescriptions very bulky as we need to fold them and time consuming to cut and fold to fit.
