Attach file to patient notes Merged

Quite frequently, documents are sent from doctors pertaining to particular patients. It would be very useful to have an 'attachments' feature in the "Patient Details" box where such attachments can be recorded, and linked. Minfos has this feature. Quick and central reference to important patient documentation is essential. Examples are S8 early supply authorisations and HMR reports.
